Defrost termination failure.

A defrost cycle fails to satisfy the controller's termination condition — it does not end on its temperature or time-out condition as configured. The coil is left iced or the case is held warm past its recovery window, a direct precursor to a temperature excursion.

Controller exposes defrost start/end and the termination condition/state; commissioned termination condition configured per case; feed health confirmed.

Action plan
(1) Confirm the termination failure from the controller's termination state, not from duration alone. (2) Correlate with the post-defrost recovery curve and the case's upper band — a failed termination that holds the case warm is a product-risk path. (3) Attempt a controllable remote reset/re-arm where exposed. (4) If unresolved, scope a dispatch to the refrigeration vendor with termination and recovery evidence.
Escalation
(1) Termination failure holding the case out of band → alert store and customer ops with a product-check recommendation; do not hold for the MBR. (2) Recurring failures on the same case → flag as a controls/hardware review candidate.
Prevention
(1) Commission and verify the termination condition per case so a real failure is distinguished from a normal long cycle. (2) Track termination-failure recurrence per case; a repeat is a hardware/controls candidate before it becomes a loss.
